Working alongside multidisciplinary colleagues and with other agencies, including Acute and Community Trusts and Primary Care services and the voluntary sector you will support service users and carers to develop personalised care and support plans which draw on community resources in order to achieve the most positive outcomes.

The service works alongside those aged 14-35 experiencing a first episode of psychosis. This is part of an integrated secondary care service which sits in an NHS Partnership Trust, under a section 75 agreement.

If this role appeals to you, please clearly express this within your application.
