We are looking for motivated, experienced professionals embarking on a new career, or returning to work, to join us working in integrated multi-agency teams, ready to embrace change and new ways of working. If you are a Social Worker, Occupational Therapist or Nurse with experience in Adult Social Care then we would love to hear from you.

As a Social Care Discharge Co-ordinator you will work with the Rapid Transfer of Care team (RTS) and manage the discharge pathway for a population of clients moving through the acute hospital setting, working closely alongside other agencies and professionals in order to achieve this.

You will be responsible for triaging and allocating referrals to appropriate workers. You will supervise and provide complex case management advice to a team of Case Officers and Case Managers. You will also be responsible for accurate data collection and escalating any issues affecting functionality of the team to the Team Manager. The RTS team operates a discharge to assess model of care which involves patients having their assessments outside of the acute hospital environment and you will be supporting clients as they move through and beyond this pathway.

The service is increasingly accessible to the public outside of traditional office hours, as we move towards an 8 am to 8 pm, 7 days a week service. OPPD staff and managers are increasingly working on a rota basis to ensure service needs are met.
