We are looking for experienced professionals and people embarking on a new career, or returning to work, to join our Short Term Pathways Team in Medway Maritime Hospital.

If you are a Social Worker, Occupational Therapist or Nurse with experience in Adult Social Care then we would love to hear from you.

As a Case Manager you will manage a caseload of cases requiring assessment, review and on-going support make a real difference in achieving improved outcomes, leading to independent living for the service user.

This post requires an excellent working knowledge & understanding of the Mental Capacity Act /DOLS, Mental Health Act and Care Act. Please specify your interest in this opportunity within your application.

The new Older People Physical Disability (OPPD) service operates with modernised office processes and performance management systems. It is increasingly accessible to the public outside of traditional office hours, as we move towards an 8 am to 8 pm, 7 days a week service to ensure service needs are met.

The ability to travel across a wide geographical area in a timely and flexible manner at various times of the day is essential. You should note that public transport is rarely a viable option as some rural areas of Kent often have limited public transport.

These posts are subject to an Enhanced Disclosure application to the Disclosure and Barring service and a relevant professional qualification (Social Work, Occupational Therapy or Nursing) as well as up to date registration with the appropriate professional body.
